This will include support to complete plans which:
• Can be easily understood and are concise
• Are clear and detailed about the amount and type of support a child or young person will receive and how the support will help
• Demonstrate clearly the involvement of the child or young person and their carers/families
• Reflect a multi-disciplinary approach in identifying SMART, person-centred outcomes
• Clearly identify the resources available to support each child or young person
